Pain Intensity Scales (pocket-sized)

This convenient pocket tool is a double-sided, laminated, four-paneled, foldable set of intensity scales with separate guides for cognitively intact and cognitively imparied patients. Scales use a combination of progressive colors, verbal descriptions, expressive faces, and numeric ratings to enable patients to indicate their degree of pain clearly. One clinician panel provides a step-by-step guide for conducting pain/symptom severity assessments and treatment planning consistently and effectively. The other clinician panel charts specific behaviors to observe when rating pain in non-verbal or unresponsive patients.
