This 3-in-1 fax, phone, and medication charting tool helps members of the health care provider team exchange and report pertinent patient information accurately and expediently. Double sided, laminated, and foldable in half, this 8.5" x 11" card is designed to fit conveniently into a practitioner's bag or binder. One side provides a concise telephone script that prompts the clear flow of information between assessing nurses and treating MDs, NPs or PAs, as well as a report line that tracks patinet pain, treatment, and response information exchanged between members of the interdisciplinary team. The other side is a scripted fax form that enables the assessing nurse to recommend medication adjustments to the physician or nurse practitioner based on pain and symptom ratings, activity levels, patient goals, and treatment response. The fax form can be photocopied for multiple use.
